Finding the Surgeon in Endacopia

The Surgeon is one of the most enigmatic characters in Endacopia. This humanoid robot resides behind a suspicious painting in a hallway featuring two dark yellow humanoid figures. Their eyes follow Mellow through the room, but close when Mellow looks directly at them.

To reach the Surgeon, you must first defeat Clockey — one of the Surgeon's creations. After the battle, the painting falls off the frame, revealing a hole in the wall. Climbing inside leads to a bare dirt room where the Surgeon sits with their back turned, working on a fleshy creature until spoken to.

Prerequisite Alert

You cannot access the Surgeon's full question tree without first completing Ending C. Each city in the game contains one Surgeon encounter, and each meeting reveals more about their motives, abilities, and past memories.

The Surgeon describes themselves as a "freelance surgeon" who can reanimate dead family members, attach multiple limbs, and create pets out of spare flesh. Their philosophical outlook treats death and life as interchangeable states of being.

How to Unlock All Surgeon Questions

Unlocking the complete dialogue tree requires methodical progression through Endacopia's three main areas. The Surgeon appears once in each city, and every encounter builds toward Ending C.

1

Defeat Clockey in the Prologue

Clockey is a creation of the Surgeon. Defeating it causes the suspicious painting to fall, revealing the hidden passage behind the wall.

2

Find the Surgeon in Every City

Locate the Surgeon once in Timesville, Misery Town, and The Office. Each encounter provides new lore and advances the hidden ending track.

3

Complete All Area Secrets

Finish every secret in each city. In Misery Town, progress through all events up to the boss fight, then follow the trail from the lobby to find the Surgeon operating on a dead clown.

4

Trigger Ending C

After completing all secrets across all three cities, Ending C unlocks. This grants access to the Surgeon's final dialogue tree with over 60 unique questions.

5

Ask Away

Return to the Surgeon and input any question. Answers are not case sensitive, and question marks are automatically appended after asking.

Input Format

Questions do not need exact capitalization. Typing "god" works the same as "God." However, some questions require specific phrasing — for example, asking "rodger" does not work, but "Who is Rodger?" does.

General Questions and Answers

These questions cover the broadest lore reveals in Endacopia. They address the nature of the world, the ritual, and Mellow's purpose.

QuestionSurgeon's Answer
What is the meaning of life?Maybe we'll find out on the other side.
What should I ask?Anything that comes to mind.
Do you love me?Of course. As much as my limitations allow.
Did you use me?We all did what we wanted to.
Are we parasites? Some are negative influence, some positive. You have been one of the latter.
Are we friends? We're friends, given the circumstances.
Why am I here?Coincidence, cosmic interference, fate. You were swallowed by God, just like everyone else.
What is going on?I'm performing a sacred ritual. I refuse to have this world die. We will sacrifice and have my bride as a vessel.
Where are we?You're in the heart of the beast. This place changes constantly.
What is this house?A house, just as any other. Abandoned on the surface. Henry found shelter in it.
Do I matter to you?Of course. In another life you might as well have been my very own son.
Will everyone be okay?Anyone not involved in the ritual is safe. Life will peacefully continue for them.
What will happen to me?We'll be consumed soon. Don't worry. It'll be beautiful.
Can I leave?I'm afraid this is the end of the line for both of us.

Is there an afterlife? One can hope.
What is the ritual?A mixture of physical procedures, paranormal synchronicities, and faith. God is dying, but not for long. With my bride as its new host, we will begin its new cycle.
What miracle?Something inexplicable by science. For years I've tried to bring the love of my life back. The missing piece was divine intervention.
What is Endacopia? To look within. A medical procedure where you travel down a patient's throat to evaluate the inside of their body. You've performed it three times through a toilet.
Did I do good? You did an amazing job.
Key Revelation

The word "Endacopia" itself is a medical term meaning to look within — specifically a procedure traveling down a patient's throat. Mellow has performed this three times via toilet, which recontextualizes the entire game's structure.

Character Identity Questions

One of the most compelling aspects of the Surgeon's dialogue is how it illuminates other characters. Below are the answers organized by category.

Who is Genesis-01?An outdated sentient computer designed to assist the company when they were a printing operation. Left abandoned after the founder went missing.

Who is Ms. Mania?A manic employee who hasn't noticed her family missing. Distracted by work.

Who is Diana?A sleep-deprived employee. Nothing wrong with them. Some people coast through life differently.
Who is Gerald?A gambler with a harmless green skin condition. Used to work nine-to-five until addiction took over. Right where he wants to be.
Lore Significance

The four clowns in Misery Town represent the Surgeon's most revealing work. They were comatose victims repurposed to fuel the new God. This confirms that the Surgeon's experiments directly feed into the ritual described in Ending C — science alone was not enough, and paranormal forces were required.

The Surgeon's Songs and Where to Find Them

The Surgeon expresses their philosophy through four distinct musical numbers scattered across Endacopia. Each song reveals deeper layers of their motivations and past.

Beautiful Creature

Location: Behind the suspicious painting in the prologue, after defeating Clockey. Ask "Who are you?" Theme: Love, creation, and the combination of two adored people into one being.

Dependency

Location: The Office, during normal progression when the Surgeon appears. Theme: Breaking chemical chains, overcoming obstacles, and the struggle between artificial and natural emotion.

Perfectionist

Location: Misery Town, after completing everything up to the boss fight. Follow the trail from the lobby. Ask "Will the patient be okay?" Theme: Modular existence, the ship of Theseus paradox applied to the body, and what happens when the captain is ill.

Bury or Die

Location: Timesville graveyard, when the Surgeon appears grave robbing. Ask "Should you really be doing that?" Theme: Natural law, obligation, replacement, and the ruthless cycle of burying failures.

Song Hunting

Each song can only be triggered once per playthrough at its specific location. If you pass through an area without triggering the song, you may need to wait until your next run. The "Perfectionist" song contains the game's most explicit philosophical metaphor — comparing bodily replacement to a ship whose boards, sails, crew, and nails are all replaced until only the question of the captain's health remains.

Ending C Preparation Checklist

Ending C Completion Requirements:

  • Defeat Clockey and enter the Surgeon's room behind the painting
  • Find and speak to the Surgeon in Timesville (graveyard)
  • Find and speak to the Surgeon in Misery Town (after boss progression)
  • Find and speak to the Surgeon in The Office (during normal progression)
  • Complete all area secrets across all three cities
  • Trigger Ending C to unlock the full question dialogue tree
  • Ask all available questions to uncover the complete lore
Missable Content

The Surgeon's songs are location-specific and tied to story progression. If you advance past a city without triggering the relevant song, you cannot hear it again until a new playthrough. Prioritize song triggers before completing each area's main objectives.

Frequently Asked Questions

Q: How many Surgeon questions are available in Endacopia?

There are over 60 unique questions you can ask the Surgeon after completing Ending C. They are divided into categories: General, About Mellow, About the Surgeon, Timesville, Misery Town, and The Office. Questions are not case sensitive and do not require question marks in your input.

Q: Do I need to complete Ending C to ask the Surgeon questions?

Yes. The full question dialogue tree only unlocks after completing Ending C. Throughout each run, you can find the Surgeon once per city, which builds toward the ending. Only after completing all secrets and triggering Ending C does the final question interface become available.

Q: What is the most important revelation from the Surgeon's answers?

Several major reveals stand out: Mellow's real name is Rodger, he died from blunt force trauma by mallet at age nine, and he may have intentionally jumped from his father's truck rather than fallen. Additionally, Endacopia is a medical term for looking within a patient's body, and the entire game takes place inside a dying god that the Surgeon is trying to save.

Q: Are there any questions that require specific phrasing to work?

Yes. Most questions work regardless of capitalization, but 'rodger' alone does not trigger a response — you must type 'Who is Rodger?' Additionally, some questions have alternate phrasings that work, such as 'What is going on?' and 'What's going on?' both producing the same answer.
